Instructions for Downloading, Completing, Saving & Printing

  1. Download the file first: Always save the PDF to your device before typing. In-browser PDF viewers frequently fail to save form field entries.
  2. Verify saving in your reader: Open the downloaded fillable PDF in a dedicated PDF reader (such as Adobe Acrobat Reader, Apple Preview, or a compatible desktop application). Enter a quick test character, save the file, close it, and reopen it to confirm that your application preserves filled form fields.
  3. Keep an untouched master copy: Use Save As to maintain an empty original copy and a separate, working completed copy.
  4. Printing instructions: When printing either edition, set page scaling to Actual Size or 100% in your print dialog. Select the Letter or A4 edition that corresponds to the paper loaded in your printer tray to prevent awkward margins.
Mobile & Third-Party Apps Note: Mobile OS viewers and built-in preview widgets handle form fields inconsistently. If interactive form fields fail to respond on your mobile phone or tablet, download the printable edition or complete the fillable PDF on a desktop workstation.
Voluntary Methodology Research

Share Your Experience and Results

Help ProComm evaluate and refine the performance dip recovery methodology. By sharing your experience, difficulties, and measured results, you contribute to identifying which adjustments work best under specific workplace disruptions.

Plain-Language Research Disclosure

What we collect: Your chosen practice frequency, ratings of exercise clarity and usefulness, difficulties encountered, and your selected single performance measure (e.g., acceptable output, handle time, or quality score) with its self-reported value and observation period.

Why we collect it: To identify effective exercises, uncover points of confusion, and measure realistic outcome recovery patterns across different working environments.

Completely voluntary: Participation is 100% optional. You may skip any question you do not wish to answer. Your access to worksheet downloads and book materials is completely unaffected.

Aggregate reporting: Results are analyzed and published strictly in aggregate form (e.g., grouped by comparable measure). We never publish individual stories or quotes without separate explicit written permission.

Withdrawal & Deletion: Upon submitting, you receive a private Participant Reference code (e.g., REF-XXXX). You can use this reference code at any time to submit a follow-up or to permanently delete your responses. Deletion immediately and permanently erases your submission from active production storage and operational databases without archiving.

Note: Information already aggregated into published statistical benchmarks cannot be retroactively separated after publication.

Online Feedback Collection Temporarily Paused

Online submissions are temporarily paused while administrative database provisioning and credential rotation are completed. All four worksheet PDF editions above are fully active, unrestricted, and ready to download.

If you have observations or notes you wish to submit directly in the interim, please email our research desk at research@procommperformance.com.

Privacy Notice: Do not include your name, employer name, customer identities, confidential work records, or sensitive personal reflections in any text field.

Thank You for Contributing to the Research!

Your response has been securely saved.

Your Private Participant Reference:

REF-XXXX

Important: Keep this code confidential. It operates as a private secret credential: possession of this reference enables you to record follow-up results or permanently withdraw and delete your data at any time. It is never made public or used for identity verification.

Indicate which stage of performance you are recording. Historical memories are categorized separately from prospectively measured baselines.
Describe the situational trigger (e.g., new software system, client policy shift, role promotion, reorganizing workflow).

Performance Measure Tracking

Choose one primary metric to observe. To ensure sound evidence, results are compared only when the metric definition and unit are consistent.

Your internal feeling of capability, distinct from the objective metric.
